Intranasal insulin (I-insulin) is a form of insulin that is a spray and inhaled through the nose. The insulin then travels to receptors in the brain (R).
Insulin receptors are located in the olfactory bulb, hypothalamus, hippocampus, cerebral cortex, and cerebellum (R).
Insulin signaling makes new synapses, and it can improve spatial memory (R).
Other pros include: it does not affect blood glucose levels (R), it prevents dopaminergic neuron loss (R), decreases hippocampal neuroinflammation (R) and improves memory in young, healthy humans (R).

1) I-Insulin Improves Brain Function
Patients with Alzheimer’s disease usually have low brain insulin signaling.
In adults with dementia or Alzheimer’s disease, intranasal insulin improved delayed memory. It also helped preserve general brain function (R).
Daily insulin treatment also increases verbal memory and improves attention (R).
In both young and postmenopausal women, insulin administration enhances working memory tasks (R).
The rapid-acting insulin analog insulin aspart (ASP-I) combined with intranasal insulin administration can improve word list recall even more than the normal insulin by itself (R).
Four months of intranasal insulin treatment improves brain functions in Alzheimer’s patients. However, the results depend on the gender of the patient. Both men and women had improvements in brain function when taking 20 IU of insulin, but only men benefited from the 40 IU dosage (R).
Brain insulin deficiency and insulin resistance can contribute to Alzheimer’s disease. Normal insulin administration in non-diabetics can cause low blood glucose. However, intranasal insulin can directly deliver insulin to the brain and help treat Alzheimer’s while avoiding side effects (R).

In overweight and obese patients, insulin action is impaired. This may reduce the inhibitory control, which will lead to overeating (R).
Brain insulin can cause anorexic effects in humans and animals. Intranasal insulin increases insulin activity in the brain, which can stop overeating and obesity (R).
Insulin and cortisol regulate energy homeostasis and appetite. Intranasal insulin application to men affects blood flow in the brain and helps regulate eating behavior (R).
Insulin can also help regulate fat metabolism. Intranasal insulin suppresses free fatty acid levels. This can help control the breakdown of fats in healthy humans (R).
In normal-weight men, intranasal insulin administration reduces body fat (R).
8 weeks of intranasal administration in men lead to decreased body weight, fat, waist circumference, and fat levels. However, it did not have the same effects in women (R).
3) I-Insulin May Help Treat Diabetes
Intranasal insulin moderates food intake and activity in the brain. It can help treat excess liver glucose production in diabetes (R).

- Chronic I-insulin treatment has stronger effects than acute treatment (R).
- I-insulin also affects heart rate variability (increased high-frequency band/parasympathetic activity indicating that vagal outputs are involved) (R).
- Treatment with 20 IU of insulin improved delayed memory (P < .05), and both doses of insulin (20 and 40 IU) preserved caregiver-rated functional ability (P < .01) (R).
- The insulin-treated group retained more verbal information after a delay compared with the placebo-assigned group (p = 0.0374). Insulin-treated subjects also showed improved attention (p = 0.0108) and functional status (p = 0.0410) (R).
- Compared with placebo, the immediate nasal administration of insulin raised diastolic BP (12.21% +/- 5.10%; P < .05), mean arterial BP (10.81% +/- 4.32%; P < .04), and systolic BP (9.53% +/- 4.66%; P < .08), whereas MSNA and heart rate were unaffected (R).
- Additionally, the insulin-treated men lost 1.28 kg body weight and 1.38 kg of body fat, and their waist circumference decreased by 1.63 cm. Plasma leptin levels dropped by an average of 27% (R).
